The second thumb I'm seeing is between the thumb and the index finger on the humans left hand.

... Or maybe I'm dumb and the human is actually holding the gun with both hands. That would explain the skin-coloured area between the index finger and the middle finger as well. Welp. Apologies for the mild confusion.
