Just like it says: a person (or animal, or anything with a mouth) is foaming at the mouth. This is traditionally a symptom of rabies (the disease, not the artist), but is also used in artwork to indicate poisoning, insanity, extreme anger (as in the expression "a frothing rage"), or having been interrupted while brushing one's teeth. Not to be confused with drooling or having a milk mustache.
