Iowa means that if a bed has two pillows, and only one person in the bed, that she'll take that as an open invitation for someone else to join.
It's the same sort of logic as "if the seat next to you at the bar is empty, then that means it's OK if I sit next to you and start flirting," just WAY more presumptuous/intimate.
